The Learning Cycle
Not a gate, but a rhythm that runs through all of them. Reviews happen at fixed moments: when a gate closes, at the end of an increment, after a steering committee, at a business-need review, at the end of a rescue and at programme close.

The same gates serve both. A new programme moves through them in order; a programme in trouble starts with a diagnosis and a short rescue sequence.
